To obtain certified copies of vital statistic records, please download the Vital Records Request PDF, complete the request form and include the following information address to:

  • Completed Vital Records Request Form
  • Check or money order made payable to the Borough of Ship Bottom
  • Stamped, self addressed envelope
  • Proof of identification (such as a copy of a drivers license)
  • Proof that you are entitled to the document

Charge for copies is $15.00 for the first copy and $5.00 for each additional copy.

Marriage License/Civil Union Application

**Marriage/Civil Union Applications are by appointment only. Please call ahead of time to schedule any time Monday – Friday, 10am – 4pm.**

To obtain a marriage license, please download the Application PDF, complete the form and provide all of the following requirements when you apply.

  • Completed Marriage License/Civil Union Application
  • Proof of Identification
  • Proof of Address
  • A Witness who knows both applicants
  • $28.00 application fee in the form of cash, check or money order made out to: The Borough of Ship Bottom

Note: There is a 72 hour waiting period after applying for the marriage license.
